PSAL Edison Team Tryouts

In order to attend ANY pre-tryout or tryout, the paperwork must be in before the tryout date. Send any completed paperwork to me at [email protected] for review. Mr. Favilla will review the paperwork and let you know if you are cleared to attend/participate. Bring all paperwork on the first date of pre-tryouts or tryouts that you attend. You will not be permitted to attend/participate if you do not bring the hard copies.
You must be cleared before you attend/participate in any team sports-related activity.
You will need the following forms listed below:
2. PSAL Parental Consent Form with all the initials marked by your parents, signed and dated
3. Current PSAL Medical Form (Completed with Doctor's Stamp)
